CALL FOR ABSTRACTS – Second National Complex Needs Conference

The Public Health Association of Australia (PHAA) – in conjunction with the National Complex Needs
Alliance (NCNA) – invites you to participate in the second Australian conference to showcase successful
programs/approaches in addressing complex needs – with the broader purpose of identifying what works
and how.

Abstract submissions close 12 June 2015.

Abstracts are being invited under the following session themes:

  • Prevention and social determinants of health
  • Comprehensive case management and/or care coordination – service providers working together
